Terms and Conditions

Liberty Kids After School Care has a number of policies that set out the procedures that are in place for the care of the child/ren who attend. We recommend that you read these. By completing the Liberty Kids After School Care online enrolment process, through AimyPlus, you have agreed that you will abide by all policies & procedures and the terms and conditions as set out below.

IMPORTANT: Should parents/caregivers have any queries or concerns regarding the following Terms and Conditions, please contact the Liberty Kids After School Care Management Team prior to completing your online enrolment.

• I will notify the Liberty Kids After School Care Management Team of any changes to enrolment information when it happens.
• I give permission for Liberty Kids After School Care staff to arrange any necessary urgent medical treatment at my cost.
• I agree to pay fees as stipulated in the Parent/Caregiver - Fees Policy.
• I give permission for my child/ren to be collected from school, or designated pick up point, by Liberty Kids After School Care staff. This can include by van or by foot.
• I understand that information gathered may be viewed by Te Kāhui Kāhu Social Services Accreditation staff.
• I agree that in Term 1 and Term 4 my child/ren will be required to wear a hat outside for sun protection.  No hat, no play when in the sun.  I understand that SPF 30+ Broad Spectrum sunscreen will be available, but if my child/ren requires special sunscreen, I will provide this and notify the Liberty Kids After School Care team.
• I agree that my child/ren will be dressed appropriately for the weather and play.
• I give Liberty Kids After School Care permission to take and use photos of my child/ren. This could include, but is not limited to, our website, newsletter, Facebook page, Instagram and for advertising and displays.
• I give permission for my child/ren to be transported to an appropriate emergency location in the event of any emergency.
• I agree that in the event that my child/ren will be absent, for any given reason, I am to contact the centre in line with our Fees policy.
• I agree that I as parent/caregiver may be liable for replacement costs of equipment or property damaged by my child/ren.
  

If there is concern for the safety of a child, or other family member, referrals may be made to the appropriate agencies.

All care will be taken to provide supervision of children attending the programme in accordance with programme policies and procedures.

Privacy Act 2020: The information that you have supplied is necessary for the safe and effective operation of the OSCAR programme. As required by the Privacy Act 2020, we follow strict procedures when storing and using the information that you provide. We are committed to keeping personal information secure and you have rights of access to, and correction of, personal information that we hold about you. We will not hold your Personal Information and other data for longer than is reasonably required for the purposes for which we may lawfully use that Personal Information or data. Following that period (or following such longer periods that we may be required by law to hold Personal Information) we will delete your Personal Information, or mask or anonymise your Personal Information so that it can no longer be used to identify you.  For more information regarding how your information is managed, please see our privacy policy.
